Reflecting on 2025: Our Annual Report is Now Available

We are pleased to share our Annual Report for 2025, reflecting on a year of progress, partnership and growth across our work to support, educate and advocate for people living with or affected by cardiovascular disease.

In 2025, we continued to strengthen our patient-centered approach by creating practical resources, working closely with local patient organizations and taking important steps toward more personal, peer-based support.

Key 2025 Highlights

  • 17 new educational resources released, including Go-To Guides, Discussion Guides, Patient Journey infographics and whiteboard animations.
  • 59 language versions produced across English, French, Flemish, German, Italian and Spanish.
  • 18,400 printed resources distributed through hospitals, clinics, patient organisations and community settings across Europe.
  • 8 million campaign impressions generated through five major awareness campaigns covering cardiomyopathy, heart failure, secondary prevention of stroke, amyloidosis and cholesterol.
  • Two peer-support programmes advanced through the Welcome Home model in France and Northern Ireland.

From education to peer support

Education remained at the heart of our work. We released 17 new resources, including Go-To Guides, Discussion Guides, Patient Journey infographics and whiteboard animations.

These materials covered topics including cardiomyopathy, heart failure, amyloidosis, cholesterol, stroke and secondary prevention, chronic kidney disease and gene therapy.

We also expanded access through our multilingual Resource Hub, which now brings together educational materials across 12 condition-specific pages in six languages. In 2025, our European community included 4,268 members.

In France, we worked with the Ligue Contre la Cardiomyopathie to adapt Welcome Home Hypertrophic Cardiomyopathy for people living with HCM.

In Northern Ireland, we partnered with Heart Failure Warriors Northern Ireland to co-design a locally tailored Welcome Home Heart Failure programme, with the Southern Health and Social Care Trust identified as the first pilot site.

Strengthening advocacy and partnerships

Advocacy continued to be an important part of our work in 2025. During the World Health Assembly in Geneva, we co-hosted a forum on cardiovascular–kidney–metabolic diseases with partners from across the patient, clinical and policy communities.

We also contributed to European policy discussions on cardiovascular prevention, heart failure diagnosis and management, and the role of blood health in future cardiovascular health planning.

Our partnerships with patient organisations in Belgium, France, Northern Ireland, Italy and Ireland helped keep this work grounded in local realities and patient needs.
